What the job involves Your role involves providing high-quality, person-centered support within the Homecare/Reablement service. You will assist individuals in their daily personal and practical care needs to help them live independently in their own homes. This includes reablement assistance, personal care, medication prompting, food preparation, and contributing to service users' health and safety plans. You will maintain detailed records of care activities and liaise effectively with individuals, their families, and other healthcare and social service professionals. Your work schedule may include nights, evenings, and weekends, with an expectation to be flexible to meet service demands.
