Job Purpose
The Care Coordinator plays a vital role in ensuring the smooth and efficient running of daily operations. This includes coordinating rotas, managing care delivery, supporting staff, and ensuring compliance with regulatory standards. The role requires excellent organisational skills, strong communication, and the ability to respond effectively to the changing needs of clients and the service.
Key Responsibilities Operational Support & Rota Management
* Plan and manage weekly rotas, ensuring all care visits are allocated and covered efficiently.
* Monitor daily service delivery and respond promptly to last-minute changes or emergencies.
* Support the smooth start-up of new care packages, including scheduling, carer allocation, and ensuring appropriate documentation is completed.
On-Call & Emergency Support
* Participate in the on-call rota to provide out-of-hours support to carers, clients, and families.
* Respond to emergency situations, ensuring continuity of care and effective problem-solving.
Team Supervision & Quality Monitoring
* Provide guidance and support to care staff, ensuring clear communication and effective task allocation.
* Carry out regular spot checks to monitor staff performance, care delivery standards, and client satisfaction.
* Assist with staff supervisions, appraisals, and team meetings.
* Promote a positive team culture and support staff well-being.
Client & Family Liaison
* Act as a point of contact for clients and families, ensuring good communication and trust.
* Visit families to discuss care needs, provide updates, and handle concerns with empathy and professionalism.
* Support client feedback and assist with complaint resolution.
Compliance & Documentation
* Maintain accurate and up-to-date care records in line with GDPR and CIW regulations.
* Support quality assurance processes and ensure compliance with safeguarding, care planning, and risk management standards.
* Keep up to date with changes in legislation and best practices.
Recruitment & Training Support
* Participate in recruitment activities including interviews and onboarding of new staff.
* Support staff induction, mandatory training, and development tracking.
Administrative Tasks
* Assist with daily reporting, monitoring staff hours, and basic budget tracking.
* Ensure all documentation and electronic records are accurate and completed on time.
Required Experience
* Previous experience in health care, nursing, or domiciliary care settings.
* Experience in rota planning and coordinating staff schedules is desirable.
* UK driving licence is essential – must be able to travel between service locations.
