To provide a specialist clinical pharmacy service to patients within the Emergency and Integrated Care (EIC) Division. To ensure the safe, effective, legal and accurate use of medicines within the speciality and to support and supervise less experienced pharmacists on rotation.
To deliver on objectives for medicines optimisation research in line with departmental priorities and initiatives supported by National Institute of Health Research Collaboration for Leadership in Applied Health research and Care in Northwest London (NIHR CLAHRC NWL).
* Clinical Practice
* Be responsible and accountable for the clinical pharmacy service provided to patients
* Leadership
* Act as a role model for senior pharmacist, junior pharmacists, and trainee pharmacists in the rotation.
* Management of service
* Ensure national and local agendas are delivered.
* To provide clinical pharmacy services for patients admitted to medicine wards, acute medical wards, Ambulatory Care and Emergency Department in accordance with the priorities set by Associate Chief Pharmacist.
* Monitor drug expenditure within the directorate and to participate in the negotiations of contracts as appropriate with the aim of saving agreed targets savings each year.
* Identify changes in prescribing practices that may impact on the drugs budget, including new prescribing practices (horizon scanning).
* Evaluation of Service
* Monitor clinical pharmacy in order to ensure safe, clinically effective and cost efficient use of drugs in these patients.
* Research and Service Development
* Manage agreed improvement work streams to deliver medicines optimisation improvements within pharmacy and the wider medicine directorate.
* Education and Training
* Participate in the training and supervision of junior pharmacists, trainee pharmacists and pharmacy undergraduates in providing and improving the clinical pharmacy service to medical patients.
