Clinical Pharmacist - Independent Prescriber
ReMind UK, an independent charity focused on neurodegenerative diseases, seeks a Clinical Pharmacist with independent prescriber qualification to manage medication for patients with Alzheimer’s disease or Lewy body dementia. You will conduct telephone consultations, oversee medication titration, and provide advice on pharmacological options.
Main Duties
* Undertake medication reviews for patients on dementia medication, including repeat prescriptions.
* Oversee medication titration and stabilization, handing prescribing over to the primary healthcare team according to ReMind UK protocols.
* Answer patient queries and concerns, adjusting prescriptions as necessary.
* Prescribe cost‑effective medication regimens.
* Advise primary healthcare teams on deprescribing or substitution recommendations for cognition‑affecting drugs.
* Liaise with primary teams regarding patient contacts and medication dose changes.
* Maintain accurate medical records.
Professional & Educational Responsibilities
* Adhere to all legal and statutory regulations of pharmacy practice.
* Comply with mental health legislation and General Pharmaceutical Council (GPhC) standards.
* Follow ReMind UK policies, SOPs, and contribute to their upkeep.
* Educate patients, carers, and staff; provide presentations and support courses.
* Engage in continuous professional development and training of new staff.
Qualifications
* Relevant Pharmaceutical degree (MPharma).
* Registration as a pharmacist with the General Pharmaceutical Council.
* Independent prescriber qualification (GPhC).
* Proven experience in dementia medication reviews, deprescribing, and telephone consulting is desirable.
Recruitment Information
Eligibility: Applicants must be eligible to work in the United Kingdom and hold current UK professional registration.
Salary: £39,000 to £44,500 per annum pro rata. Contract: 6‑month fixed term, part‑time with flexible working. Location: The ReMind UK Centre, Combe Park, Bath BA1 3NG.
