Job overview

An exciting opportunity has arisen to join the Home First Therapy team in Buckinghamshire.

The team is still in its infancy of becoming a substantive service and this provides team members with the opportunity to influence and contribute to growing and shaping the service.

We are looking for motivated and skilled individuals to join the team, which supports patients that have recently been discharged from hospital, to regain their independence and remain in their own homes.

The ideal candidate will be able to take a holistic and person centred approach to supporting patients to recover from their recent hospital admission, and to achieve their short term goals, maximising independence.

The role will include visiting patients in their home, to perform assessment and treatment for patients with complex needs and within the expected timescales. Additionally, the successful candidate will be required to provide advice, guidance and information to health and social care professionals.

Main duties of the job

To perform assessment and treatment for patients with complex needs, and to provide advice, guidance and information to health and social care professionals, carers and relatives and other non-professionals.

To undertake all aspects of clinical duties as an autonomous practitioner.

To participate in continuing professional and service development and contribute to organisational objectives.

Education, Qualifications & Training

Essential criteria

1. Current registration with Health Professions Council
2. Diploma /Degree in Occupational Therapy
3. Evidence of CPD in the form of a detailed personal development portfolio

Desirable criteria

4. Specific formal courses appropriate to post speciality

Experience

Essential criteria

5. Experience of at least 18 months in a junior rotational Occupational Therapy post
6. Previous experience in core Occupational Therapy specialties
7. Attendance at post graduate courses in related fields and demonstrates reflective learning
8. Experience of managing own caseload and prioritizing
9. Ability to work and interact effectively within an interdisciplinary team and contribute to decision making
10. Evidence of commitment to continuing professional development, and reflective practice
11. Knowledge and application of evidence based practice and clinical reasoning
12. Establish and maintain CPD Portfolio

Desirable criteria

13. Knowledge of areas of influence of psychosocial and cultural factors on health/illness behaviours
14. Knowledge of Discharge pathways

Skills, Abilities & Knowledge

Essential criteria

15. Able to apply a range and depth of knowledge to the specialist clinical area of post
16. Able to make sound clinical decisions based on knowledge/ experience and utilize advanced clinical reasoning skills
17. Able to carry out a range of assessments and procedures in order to make clinical assessments and diagnosis
18. ble to carry out manual therapy techniques and therapeutic handling, requiring skillful and strenuous physical effort during the day
19. Awareness of limitations of own professional abilities
20. Clear, concise verbal and written communication to ensure liaison with multi disciplinary team, patients and carers to ensure records and reports are accurate, legal and in legible manner (in English)
21. Ability to integrate, participate and contribute as an effective member of a team
22. Ability to self direct towards the attainment of clear goals
23. Ability to prioritise tasks and functions against well reasoned standards.
24. Positive response to constructive criticism
25. Ability to plan, manage and organise required tasks within available resources (including time)
26. Use of and/or knowledge of POMR record keeping
27. Knowledge of Current related legislation (Care Act, H&S, Data P. Act, etc)

32. Able to fulfill the physical demands of the role subject to reasonable adjustment under the terms of the Disciplinary Discrimination Act 1995
33. Able to be empathetic and handle difficult or emotional situations, using judgement to determine optimal intervention
34. Ability to participate in weekend working
35. Flexible to meet the needs of the service
36. Full UK Driving Licence
37. Availability of own car for work use

PPE requirements: Buckinghamshire Healthcare NHS Trust requires all colleagues to wear appropriate personal protective equipment (PPE) in accordance with our infection prevention and control procedures.
