A Support Worker Team Leader is required to work with a 10-year-old and their family, undertaking the day-to-day management of the support package. You will be working with their MDT and school to ensure that their needs are met and to maximise their potential, as well as assisting them in their home and the community.

About the role:
Working Hours: Average 37 hours per week (30hrs clinical / 7 hrs admin)
Shift Pattern: days, evening, weekends. Sleep-ins, waking nights

Pay rates:

Weekdays - £25 per hour
Weekends - £27 per hour
Sleep-in - £14 per hour
Your responsibilities as Support Worker Team Leader:

Providing flexible, person-centred support in line with the client’s needs, plans and risk assessments, ensuring safety, wellbeing, privacy and confidentiality at all times.
Liaising effectively with the client, family, Case Manager and MDT to maintain consistent care, contribute to rotas, attend meetings and communicate concerns, incidents or changes promptly.
Maintaining accurate records including daily notes, handovers, timesheets, financial records and incident documentation, ensuring all paperwork is completed and submitted as required.
Managing and overseeing all aspects of client medication, including PEG care, storage, recording and ensuring adequate supplies are maintained.
Supporting and supervising the wider team by completing supervisions, monitoring training compliance, booking courses and contributing to staff development.
Updating support plans, risk assessments and related documentation when requested, and undertaking any other reasonable duties in line with the client’s needs.
We’re looking for a Support Worker Team Leader with:

Proven experience in a Team Leader role supporting clients with complex needs, including cerebral palsy, within a multidisciplinary team environment.
Strong experience following MDT therapy and treatment plans, working in partnership with families, professionals and external agencies.
Experience in staff supervision, rota management, documentation development and the day-to-day coordination of care packages.
Competence in medication administration, including PEG management, supported by a current medication certificate and relevant Health and Social Care qualification (Level 2/3 or equivalent).
Effective written and verbal communication skills, strong interpersonal abilities, confidence working independently and within a team, and sound use of initiative.
Full UK driving licence with the ability to drive a WAV, flexibility to work varied shifts including nights and weekends, and physical capability to meet moving and handling requirements.
