This is an exciting new role in our practice to join us as a Home Visiting Practice Nurse within our SMART Team. We are looking for around 15-16 hours per week on a fixed term contract for 2 years.
The aim of the team is to proactively address the needs of frail patients in the community to reduce health inequalities, including improved access to early dementia screening and diagnosis.
Main duties of the job
The main duties of this role will be to deliver nursing care to housebound patients and those in our care homes, and to assist and perform routine tasks related to patient care as directed by senior nursing staff and GPs.
