This role is central to ensuring safe, timely and effective coordination of care across our community health pathways.As an autonomous practitioner within a multidisciplinary team, you will provide expert telephone‑based clinical triage, using your clinical knowledge and understanding of urgent and community services to assess, prioritise and direct referrals appropriately.
