Job summary
We are currently recruiting for a full time innovative Pharmacist position hosted by the
Pharmacy Department at University Hospitals Birmingham (UHB, employing organisation)
for the Solihull Community Services role and Virtual Wards.
Solihull Community Services Role:
1) To partake in specialised clinical pharmacy duties within the pharmacy community
services team. This includes supporting medicines management, medication reviews,
medicines optimisation and clinical governance within Solihull community based services
including Intermediate Care.
2) The post holder will assist in the delivery of education and training, developing
/updating procedures and protocols along with medicines usage information.
3) To promote the safe and effective use of medicines through the provision of pharmacy
services and advice to patients, carers and other healthcare professionals as part of a
multidisciplinary team.
Virtual Ward Role:
1) To be an integral member of the multidisciplinary team developing and delivering
services and clinical pathways to virtual wards.
2) Aim to reduce referrals and attendance at the Emergency Department; support
proactive safe and timely discharge of complex inpatients to community care; and deliver
care based around individual patient needs.
3) To lead and develop clinical pharmacy services to the virtual ward which provides an
alternative clinical pathway for respiratory and frailty patients and those with long term
conditions
Main duties of the job
To provide a clinical pharmacy service according to trust agreed standards including:
Community Services Role
*Review and optimise medicines for patients within the intermediate care and falls services. Identify medication errors by undertaking medicines reconciliation.
*Provide advice on administration of medicines, dosage, ADRs, cautions and monitoring required. Educate patients and aid with the discharge planning process.
*Review current working practices to ensure optimal services are provided and record professional activities through clinical interventions.
Virtual Wards Role
*To provide clinical specialist input to virtual wards and be an integral member of MDT. Attend ward rounds and MDT meetings.
*Optimise medications for patients and make proactive interventions in patient's therapy by providing evidence-based information on medicines related issues
*Encourage compliance with the national, regional, and local clinical guidelines and the prescribing formulary to promote rational and cost-effective prescribing
*Provide advice and education to patients and/or carers on the correct use of medication. Proactively signpost and refer patients to external agencies for further support if necessary
*Aid with production of policies, procedures, guidelines, and PGDs to assist in the smooth delivery of medicines optimisation to the Directorate
* Identify and prevent hospital admissions through audits and patient reviews

Qualifications

1. *Masters degree in Pharmacy (or equivalent)
2. *Registrant of the General Pharmaceutical Council (GPhC)
3. *Completion of first year of Post-graduate clinical pharmacy qualification or equivalent experience

4. *Non-medical prescribing qualification

5. *Experience of teaching/presenting to other healthcare professionals/patients
6. *Knowledge of national initiatives and guidelines, NICE

7. *Post-registration hospital pharmacy experience in clinical pharmacy services
8. *Previous experience in acute and emergency medicine
9. *Possesses knowledge of available resources to answer Medicines Information queries.
