Job Overview
University Hospital Southampton NHS Foundation Trust offers a position on the paediatric long term ventilation team. The role provides specialist care for children and young people with long‑term ventilation needs, supporting patients during hospital stay, coordinating multidisciplinary care, and planning safe discharges. The role also involves working with WellChild charity to provide education and training.
Responsibilities
* Primary responsibility for provision of specialist nursing to a defined patient/client group, practising autonomously.
* Identify key worker for patient/client caseload, coordinating and overseeing therapeutic treatment and care.
* Provide specialist advice, education and support to patients, carers and other health professionals.
* Practice clinically in agreed care settings and advise and support divisions regarding overall service management.
* Develop and support patient‑centred seamless care across professional, departmental and Trust boundaries, leading nurse‑led care initiatives.
* Develop, disseminate and evaluate training programmes and resources for those caring for children LTV needs (Well Child Initiative).
* Pro‑actively manage the educational agendas within the LTV team across acute and community sectors.
* Partner with the Multi Professional Team and other Practice Educators to provide education and improve clinical nursing and service delivery.
* Promote education for LTV patients in various settings including UHS, patient homes, community services, schools, outpatients and regional hospitals.
Qualifications, Knowledge and Experience
* Registered nurse on relevant part of the NMC register.
* Level 7 Post Grad Cert (min 60 credits), Master’s Degree within relevant speciality or equivalent knowledge.
* Specialist training or qualification in area of practice.
* Evidence of leading learning in practice.
* Evidence of experience and completion of all band 6 competencies within relevant speciality.
* Specialist clinical skills and experience within specialist area.
* Evidence of change management, leadership and innovation.
* Clinical experience at Band 6 or equivalent within relevant speciality.
* Evidence of supporting colleagues to promote learning and providing an excellent learning environment.
* Evidence of supporting, managing and supervising a range of learnings in practice.
Desirable Criteria
* Counselling or advanced communication skills qualification or training.
* Act in the role of Practice Assessor or supporting progression of learners.
* Experience of nurse‑led care or clinics in own practice.
* Team leadership role.
* Registered as an independent prescriber.
* Leadership qualification or training.
