Job summary
An exciting opportunity has arisen for an Assistant Navigator in our new Personalisation and Social Inclusion Team for Adults with Learning Disabilities and Autism in Staffordshire. This is a new post within a new team and you will play a key role in developing and delivering this service.
We would like to hear from you if you have an interest in and experience of working with people living with Learning Disabilities and/or Autism, this will include supporting people during the transition from young person to adulthood, supporting their independence within their homes, neighbourhoods and wider communities.
A variety of interests and skills are essential to help inspire people who use the service to move forward in their lives; the confidence and enthusiasm to work with partners to explore opportunities in local communities, support community participation and build community capacity.
This is both an interesting and challenging opportunity and you will benefit from joining a team who are committed to providing a quality service to the Adults and Carers that we work with.
You will ideally need to be able to travel across the locality and can be based either North Staffordshire or East Staffordshire, working hours are negotiable.

Main duties of the job
You would be required to demonstrate that you are motivated, can work as part of a team, engage with people, maintain a caseload, promote independence and value people, their families and friends as essential partners
As part of this role you will be required to:
1. Work alongside colleagues, people who use the service and their carers to promote positive images of mental health.
2. While considering financial and other resources, support new and existing projects, activities and opportunities for adults in line with service and individual goals promoting social inclusion, wellbeing and Recovery; including peer support and partnership opportunities.
3. Provide appropriate assistance and support to people who use the service and their carers at any of the six localities across South Staffordshire. This will include supporting individuals to participate in mainstream community life as per their agreed care plan and personal development plan
4. Keep coherent records of all service delivery activity and achieved outcomes in line with Trust protocols; participate in audits related to service delivery and development.
5. Enable adults to maintain and develop contact with friends and the community

Social Work Learning Academy (SWLA)
On appointment, you will be enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage ( stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.

15. Level 3 Diploma in Health & Social Care or equivalent and willing to undertake additional qualifications in Health and Social Care
