Job Description:
We are seeking an experienced Occupational Therapist to join a community-based service in Cornwall. This role focuses on delivering optimal, person-centred care while supporting service efficiencies and reducing reliance on long-term care packages.
The successful candidate will work directly with adults in the community, carrying a caseload and completing regular face-to-face assessments and interventions.
Key Responsibilities
1. Undertake community-based occupational therapy assessments
2. Support individuals to maximise independence and wellbeing
3. Recommend practical, proportionate interventions to reduce dependency on ongoing care packages
4. Manage a varied caseload with regular in-person visits
5. Work collaboratively with wider health and social care professionals
Essential Requirements
6. Qualified and registered Occupational Therapist
7. Significant post-qualification experience in a community setting
8. Strong preference for experience within Adult Social Care
9. Ability to work independently and manage multiple cases
10. Willingness and ability to undertake regular face-to-face visits
Desirable
11. Knowledge of strengths-based practice and prevention-focused approaches
12. Experience contributing to service efficiency or cost-saving initiatives
