Job Title: Mental Health Practitioner – Mental Health Teams

Rate: £26.00
Liquid Personnel is currently recruiting experienced and Qualified Mental Health Practitioners to join Mental Health Teams based in Hereford and Worcestershire areas.
Responsibilities:
You will provide high-quality assessment and intervention to service users experiencing a wide range of mental health problems, promoting recovery and independence to identify appropriate pathways and treatment plans with service users. Your duties will include care planning, care coordination, managing a caseload in accordance with the Care Programme Approach (CPA), psychosocial intervention, risk assessments, and supporting with Adult Safeguarding. You will work as part of a multidisciplinary team (MDT), actively participate in, and contribute to supervision and team meetings. Additionally, you will maintain a personal awareness of legislation, policies, and procedures, particularly those relating to the Care Act, The Mental Health Act, the Mental Capacity Act, safeguarding, and self-directed support to apply a strong evidence base to ensure consistency and a high quality of service delivery.
Qualifications and Experiences:
You must have a relevant Professional registration (SWE, NMC, or HCPC), be eligible to work in the UK, hold a qualification in Social Work, Nursing, or Occupational Therapy at degree level, and have post-qualified experience. If the role is community-based, you must also hold a full UK driving licence.
