An exciting opportunity has arisen within the Rochdale West Community Mental Health Team for a Community Psychiatric Nurse / Social Worker or OT to join us in working to provide mental health care to people of working age who have moderate to severe mental health problems across a wide range of presenting conditions.

There is a strong commitment to promoting your professional development. You will be supported by clinical and management supervision and will be given the opportunity to develop and enhance clinical skills, interventions and case management through a personal development and training plan.


Main duties of the job

* To act as a Community Mental Health Practitioner for a defined caseload working with evidence based interventions
* Assess and review needs
* Implementation of care plans
* Assessment, monitoring and management of risk
* Monitor, review and evaluate care
* Liaise with all agencies, and members of the multi-disciplinary team
* Carry out clinic duties such as the Clozaril /administration of depot injections either in the clients home or in the clinics if Nurse qualified where required
* Supervise Community Support Workers / Band 5s and peers as appropriate
* To maintain accurate, up to date electronic records
* To participate in the CMHT’s duty system on a Rota basis. Be able to offer a rapid response to service users’ immediate needs, including those arising from crisis
* To work in a therapeutically informed way within the multi-disciplinary team to address the needs of service users with complex emotional disorders
* To maintain a persistent approach to engagement where there are complex mental health difficulties
* To prioritise and work flexibly with MDT in order to respond to rapidly changing clinical needs
* To promote the physical health and wellbeing

Detailed job description and main responsibilities

Main Duties and Responsibilities

* To assess the health and social care needs of individuals referred to the service, holistically, under the agreed procedures
* To conduct specialist mental health assessments and risk assessments and devise appropriate care plans and risk management plans
* To provide evidence based therapeutic interventions appropriate to the service users assessed needs
* To provide direct assistance, including education and advice, emotional and psychological support to those referred to the team with the aim of promoting re-enablement and recovery
* To promote choice and service user involvement in care planning and risk management
* To ensure practice is in line with social inclusion and recovery models
* To support and work with colleagues, other MDT members and outside agencies collaboratively and in best interests of the service user
* To liaise with and provide written and verbal feedback to clients, carers and relevant professionals (e.g. GP’s), giving due regard to consent, confidentiality and Information Governance guidelines
* To ensure all carers are signposted to access Carer Assessments
* To report concerns identified under Safeguarding Adults procedures
* To observe the legal requirements of the current Mental Health Act, Mental Capacity Act, Care Act and other relevant legislative frameworks
* To maintain accurate, contemporaneous clinical records on all service user contact, in accordance with current Trust documentation standards, and with due regard to confidentiality and Information Governance guidelines
* To complete all necessary inputting, training and documentation related to electronic record systems


Person specification


Education and Qualifications

* Registered Mental Health Nurse / Social Worker / Occupational Therapist
* Current registration with appropriate regulatory body (e.g. NMC, AHPC)
* Completion of appropriate post qualification consolidation e.g. Preceptorship or the assessed and supported year in employment (ASYE)
* Qualification in psychologic therapeutic interventions


Experience

* Substantial experience of relevant post registration practice
* Experience of managing a caseload
* Experience of working in a multi-disciplinary team setting
* Experience of working with people with a range of severe and complex mental health problems
* Experience of clinical supervision of other staff
* Experience of working within a Community setting


Knowledge

* Good working knowledge of therapeutic assessment techniques and interventions in the management of behaviour of individuals with complex mental health needs
* Knowledge of a range of screening and assessment tools and interventions utilised to support individuals with severe and complex mental health problems
* Comprehensive understanding of Safeguarding adults procedures and practice
* Comprehensive understanding of the Mental Capacity Act, Mental Health Act and Care Act
* Comprehensive understanding of roles and responsibilities under the Care Programme Approach
* Understanding of key national and local policies and drivers in relation to adult mental health service
* Knowledge of local authority and health commissioning processes


Skills and Abilities

* Ability to undertake the community mental health practitioner role
* Evidence of ability to provide specialist mental health assessment and risk assessment / management and be accountable for decision making
* Excellent written and interpersonal communication skills
* Ability to engage with and provide therapeutic interventions for this client group and carers
* Ability to prepare, record and complete relevant care records and reports to a professional standard
* Ability to coordinate and delegate the work of junior staff


Work Related Circumstances

* Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs
* Willing to carry out all duties and responsibilities of the post in accordance with the Trust’s Equal Opportunities and Equality and Diversity policies
* Appointments to regulated and controlled activities require an enhanced DBS disclosure

What happens after your application has been received?

What happens if I am offered the position after interview?

* Identity verification
* Right to work check
* Disclosure and barring service (DBS)/Criminal record check (dependent on role)
* Professional registration and/or qualification check
* Occupational health assessment
* Employment history and reference validation

All applicants external to the NHS will be required to provide their HMRC employment history to cover the most recent three years employment. This information will used to validate employment history and references as part of pre-employment checks.

What happens when pre-employment checks are complete?
