Overview
We are seeking a committed and skilled Social Worker to join the Adult Services team at the London Borough of Hackney. The successful candidate will deliver a high-quality, strengths-based social work service, supporting adults, carers, and their families to achieve positive outcomes and live independently. You will play a key role in assessments, care planning, and safeguarding, working in line with the Care Act principles and statutory responsibilities. This role requires a compassionate approach, strong case management skills, and the ability to work collaboratively within a multi-disciplinary environment.
Responsibilities
* Carry out strengths-based assessments of adults and carers in line with the Care Act and local policies.
* Develop personalised, outcomes-focused support plans that promote independence and wellbeing.
* Provide information, advice, and signposting to help people make informed decisions about their care and support.
* Lead and contribute to safeguarding adults enquiries, applying Making Safeguarding Personal principles.
* Work closely with health, mental health, and social care professionals to ensure joined-up support for clients.
* Apply knowledge of the Mental Capacity Act, Deprivation of Liberty Safeguards, and risk management frameworks.
* Maintain accurate, high-quality case records, reports, and documentation in line with professional standards.
* Contribute to legal processes, including preparing court reports when required.
* Promote equality, diversity, and inclusion in service delivery.
* Participate in reflective practice, supervision, and continuous professional development.
Requirements
* A recognised qualification in Social Work (e.g., degree, DipSW, CQSW).
* Registration with Social Work England.
* Knowledge of the Care Act, Mental Capacity Act, DOLS, and safeguarding frameworks.
* Strong interpersonal and communication skills, with the ability to build effective relationships with service users, carers, and professionals.
* Experience of assessment, support planning, and case management (ASYE applicants will be supported).
* Ability to work independently and as part of a team in a diverse community setting.
* Proficiency in IT systems for case recording and report writing.
* Commitment to personal development and best practice in adult social care.
