Overview

A 6-month secondment role has become available for a Deputy Discharge Hub Lead to support the coordinating and planning of patient transfers of care at West Middlesex Hospital.


Key Details

* Operational management, leadership and coordination of the daily activities and planning of the West Middlesex Discharge Hub.
* Work with community services in an integrated manner to improve understanding of models of care ensuring patients access the most appropriate pathways.
* Proactively plan and organise effectively at times of unexpected surges in patient activity and act as the central point of contact to deliver a coordinated and effective response for management of patient flow for secondary care and the link out to the community teams.
* Location: Based at West Middlesex with occasional travel to all sites within the system.


Responsibilities

* Support the management of the Discharge Hub at West Middlesex Hospital, coordinating and planning the transfer of patients on pathways 1, 2 and 3 in line with the home first principle for West Middlesex Hospital and Hounslow patients from other hospitals.
* Work with the patient, family, wards and community staff to determine and decide the right level of support and follow‑up assessment required to ensure a safe and timely transfer back to the community setting.
* Provide expert advice to other members of the MDT and other agencies regarding the management of transfer of care of patients.
* Help achieve national and local targets by influencing patient movement into and out of acute hospital beds.
* Lead various system‑wide projects and initiatives focused on safe discharge.
* Provide senior clinical decision‑making/expertise and direction to multi‑agency health, social care and third sector staff, enabling a culture of learning for safe outcomes.
* Ensure integrated processes are created and embedded in the recording and understanding of system‑wide data relating to discharge, aiming to improve patient experience/outcome within the transfer of care pathway.
* Escalate key areas appropriately to the relevant agencies/services in a timely manner.
* Make independent assessment and recommendations regarding course of action in complex admission/discharge planning situations.
* Support ward‑based multi‑disciplinary team members and coordinate patient flow activities.
* Communicate and implement changes to clinical pathways within the Discharge Team.
* Support and contribute actively to the training and development of other staff to improve service provision.


Qualifications & Skills

* Clinical and professional leadership experience in discharge or patient flow management.
* Strong communication and stakeholder engagement skills with front‑line clinicians, senior managers and strategic partners.
* Ability to plan and adapt strategies during unexpected surges in patient activity.
* Knowledge of community services and models of care for patient transfer.
* Experience with data‑driven discharge initiatives is desirable.
* Working knowledge of integrated care pathways within the NHS, particularly in West London.
