Overview

An exciting opportunity has arisen for a Community Mental Health Nurse position within the Recovery and Wellbeing Team based in Brentwood. The post holder will join an evolving team and will be jointly responsible for the delivery of a high standard community care, forming part of a coordinated plan of community treatment to Service Users predominantly within their own homes. The post holder carries responsibility for the assessment of coordinating care needs and their development, implementation and evaluation of programmes of care for Adults with mental health problems that are referred to the First Response and Recovery and Wellbeing Team. The post holder is responsible on a day-to-day basis to the Clinical Lead but ultimately accountable to the Team Manager for the overall service. They will be a member of a multi-disciplinary team and work in close cooperation with GPs, wards and Primary Health Care based professionals, ensuring an integrated service is provided to clients and maintaining a good inter-disciplinary working environment.


KEY RESPONSIBILITIES

* Hold patient caseloads and maintain effective individual care regarding mental, health and social care needs by: holistic assessment of client problem/needs; planning, implementation and evaluation of interventions; clear, accurate and concise documentation of client care.
* Participate in the initial assessment of clients to determine appropriateness for the Community Mental Health Service and liaise with referring agencies to identify alternative agencies as appropriate to meet the client\'s needs.
* Participate in Team and Clinical Meetings, initiating and discussing new methods/models of care and the introduction of research or trials within the area.
* Be fully aware of the importance of effective risk management, undertaking clinical risk assessment and ongoing monitoring of the client\'s mental state to evaluate risk to themselves or others.
