The Role

To develop and lead the clinical pharmacy service to the NHS Lanarkshire Hospital at Home team. To develop governance processes within the service and utilise the unique knowledge and skills base of a pharmacist to ensure the safe, effective and economic use of medicines, by the delivery of high quality pharmaceutical care. This will be delivered by leading a small team to and through collaborative partnerships so patients get the best possible outcomes from their medicines, while avoiding harm and waste.

Additional Information Specific to the Role

This exciting new role will contribute to the Hospital at Home Service across NHS Lanarkshire to ensure patients receive high quality pharmaceutical care. The post holder will become an integrated member of the consultant-led multidisciplinary team. This post holder will be required to work across all four of the Hospital at Home bases at UHW, UHM, UHH and Clydesdale. There is a requirement of the post holder to have significant relevant clinical pharmacy experience and be an established practicing independent prescriber due to the complexities of medicines procurement, prescribing and administration within the service. The Clinical Pharmacist Hospital at Home will support the post holder.

The post holder will develop the clinical governance agenda and associated systems for medicines use within the service. This will include but limited to:

1. Provision of professional and operational leadership, develop and train the other members of the pharmacy team within the Hospital at Home (H@H) service.
2. Lead and oversee medicines optimisation within H@H.
3. Provide pharmaceutical advice for the H@H service
4. Develop and review clinical protocols and medicines governance process for H@H.
5. Undertake quality improvement initiatives and where appropriate liaising with key stakeholders
6. Support with formulary compliance/awareness
7. Support with prescribing of unlicensed medicines
8. Facilitate and support processes for medicines communication at the interfaces of care e.g. On discharge to a hospital ward or discharge to the care of the General Practice via an immediate discharge letter
9. Liaise with relevant health and social care services as it applies to medicines e.g. social work, acute hospital departments, General Practice, NHSL acute pharmacy services (including off site beds), Community Pharmacy and other key stakeholders.
10. Support and facilitate governance processes associated with medicine prescribing and administration by the Hospital at Home team
11. Lead on new service developments and medicines optimisation projects
12. Delivery of relevant pharmacotherapy activities e.g. medicines reconciliation, medicines waste
13. Participation, where relevant, at virtual multidisciplinary ward rounds
14. Facilitate safe supply and administration of medicines in patients own home e.g. facilitate medication changes considering associated complexities of administration in the home
15. Liaise with community pharmacy and any other key stakeholders to facilitate safe processes for procurement of medicines for patients and clinical staff e.g. TTOs and ward stock or prescribing on HBP10.
16. Participation in domiciliary visits providing input and pharmaceutical advice to support the clinical team in delivery of care
17. Support with delivery of relevant education and training with the Hospital at Home MDT and peer learning sessions both within the service and with key stakeholders
18. Liaise and participate in the Hospital at home UK and Ireland pharmacy network

There will be a focus on optimal use of medicines to ensure a patient focused, efficient progressive, resilient, safe and effective NHSL Hospital at Home service

What we'll need you to bring

Masters degree in Pharmacy

Registered to practice with the General Pharmaceutical Council

Continuing Professional Development as detailed by the General Pharmaceutical Council (mandatory requirement)

Ability to monitor patient therapeutic regimens to ensure effectiveness and reduce toxicity.

Previous experience in delivering training and education at post-graduate level.

Demonstrable use of clinical audit to improve practice.

Demonstrate advanced level of clinical reasoning and judgement.

Excellent numeracy, verbal and written communication skills

Ability to work to deadlines and manage resources

Participation in weekend and public holiday working. This does not apply to primary care roles

Participation in pharmacy emergency duty service (if appropriate). This does not apply to primary care roles

Post graduate degree / diploma in Clinical Pharmacy or Management

Please note for all vacancies where a driving licence is required this must be a full UK/EU/EEA licence.

It would be great if you also have

Experience in the specialist area the post covers e.g. paediatrics, chronic pain

Pharmacy experience including clinical decision making with evidence of completion of/or demonstrable evidence of equivalent to a foundation level professional accreditation program

Experience in the specialist area the post covers
Evidence of progress to/completion of a higher level professional accreditation program
Evidence of research activity in either pharmacy practice or medicines utilisation

Ability to undertake the duties of pre-registration pharmacist tutor

Ability to influence senior pharmacy medical and managerial staff and the multidisciplinary team

This role will be based in University Hospital Monklands.

The working pattern for this role Mon - Fri; 9:00-5:00
