Job Overview
Cornwall Partnership NHS Foundation Trust is passionate about promoting good mental health and wellbeing. This role leads on a new service – the Mental Health Response Vehicle (MHRV) – in collaboration with SWAST. This position is open only to current NHS employees in Cornwall or the Isles of Scilly from Cornwall Foundation Trust, Royal Cornwall Hospitals NHS Trust, or Cornwall and Isles of Scilly Integrated Care Board.
Main Duties
* Provide expert advice, line management, clinical leadership, caseload management, supervision and training to the team, ensuring consistent high quality mental health care to people in mental health crisis in the community.
* Deliver expert clinical knowledge and conduct prompt biopsychosocial assessments and treatments for Crisis Hubs, HTT and SPOA patients with mental health or self‑harm issues.
* Provide direct patient care to people acutely mentally ill at home, Crisis Hub, who do not require immediate physical health care.
* Respond rapidly to referrals, conduct face‑to‑face assessments, gate‑keep admissions to hospital, and facilitate discharge from inpatient services once risk has decreased.
* Conduct urgent front‑line assessments from GP, police, ambulance, local authority and others, making clinical decisions about appropriate care pathways.
* Apply excellent risk assessment skills and a holistic approach to assessing health and social needs of people with severe mental illness.
* Refer and signpost to secondary mental health services or alternative statutory/non‑statutory agencies, working as part of a multidisciplinary team.
Person Specification – Education / Qualifications
* Current professional registration NMC, HCPC, or SW England.
* Substantial Home Treatment Team crisis resolution practitioner experience and working knowledge of crisis assessment and intervention.
* Experience in conducting clinical assessments including risk assessment.
* Professional qualification RMN, OT, or SW.
