About the Role
Shift details: Varied shifts including alternate weekends. As a Team Leader at Loxley Park Care Home in Sheffield, you will support the home management team in delivering high-quality, person-centred care with a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.
Job Responsibilities
* Lead the person-centred care planning process: implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans and ensuring they are carried out.
* Assist in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed where appropriate.
* Monitor the well‑being of each resident, being aware of any changes impacting care and care planning, and ensuring that changing needs are met and written records are amended accordingly.
* Practice safe systems of work across a range of tasks, assessment of risks and safeguarding personal safety and the safety of residents, visitors and staff.

Qualifications & Experience
* NVQ Level 3 or equivalent in care for care homes.
* Minimum of two years' experience working in a senior position with a resident group.
* Strong verbal and written communication skills.
* Demonstrated ability to lead staff and act responsibly in the absence of the manager.

* A DABS Disclosure check, fee paid by Avery Healthcare.
* Proof of eligibility to work in the UK.
