We are seeking highly motivated SPQ District Nurses to join the Integrated Care Community Teams across South Tyneside and Sunderland. You will be involved in the provision of high quality nursing assessments and be responsible for the delivery of clinical care. The ability to communicate positively at all levels will be an essential attribute of the post holder, as well as a flexible self‑confident approach to work. The post holder will have the knowledge and experience to provide a valuable contribution to the development of more flexible and innovative working both within the team and across organisations in the statutory and non‑statutory sector. In return for your commitment to team working we will offer you a comprehensive induction and preceptorship package and support in personal and professional development within the appraisal process. A recognised qualification relevant to the speciality is essential. Car driver/access to a car is essential.


Main duties of the job

The post-holder is responsible for the management of a defined caseload and for taking continuing responsibility for the development and delivery of programmes of care, being responsible for the comprehensive assessment, planning, provision and evaluation of patient care needs.

To manage and support junior staff by prioritising and delegating work appropriately, monitoring and ensuring safe working practices.

Act as a clinical leader within the integrated teams, further understanding within the team of strategies to manage risk, plan for foreseeable events, and utilise self‑care approaches.

Demonstrate a high level of clinical judgement and leadership, acting autonomously in a variety of contexts across the health and social care economy.

Qualifications

* Registered General Nurse
* Extended Nursing skills
* Teaching qualification
* Extended Clinical skills
* Ability to undertake audit
* Supplementary Prescriber


Physical skills

* Able to move and handle safely without causing harm to themselves or others


Experience

* Ability organise workload and work autonomously and within a team
* Ability to work under pressure and achieve tight deadlines in a complex / changing environment
* Complex caseload management
* Multi‑agency working experience
* Ability to undertake research
* Extensive community experience


Skills and Knowledge

* Evidence of on‑going professional development, willingness to maintain and extend skills and knowledge, and an ability to share and influence the scope and reach of the Integrated Care Team
* Excellent inter‑personal and communication skills in order to forge relationships with colleagues and others for the benefit of patient care and caseload coordination
* Expert assessment, analytical, interpretation and clinical decision making skills
* IT skills and knowledge
* Condition specific qualification e.g. Tissue Viability / Palliative Care
