Support Worker
Location: Hampshire (Winchester and Basingstoke area)
Salary: £22,932 FTE (£18,345 for 28 hour week) per annum
Hours: 28 per week (typically 10am – 6pm, Mon, Tue, Thu, Fri)
Contract: Fixed term until 30th September 2025 (likely to be extended)
Driving Requirement: Full UK licence, at least 2 years

Key Responsibilities
* Providing practical and emotional support (telephone calls, welfare checks, shopping, etc.) and guidance to service users.
* Supporting adults following hospital stays to enable early discharge.
* Responding to referrals from Clinicians and Community Health Professionals.
* Assessing needs and following support plans for individuals.
* Supporting service users after hospital discharge or to prevent readmission.
* Assisting with hospital appointments, prescriptions, and shopping.
What You Need to Succeed
* Good knowledge of NHS and Social Care services.
* Ability to improve service quality for users.
* Professionalism in handling queries confidentially and diplomatically.
* Flexibility with working hours outside the norm.
This role involves no personal care or medication administration.
