Support Worker (Children, Young People & Families) - 21 HSC Wraparound

Location: Midlothian
Contract: Flexible - Full-time, Part-time & Sessional
Salary: £17.50p/h - £19.50p/h PAYE

About the Service

21HSCs Wraparound Care and Support Service for Children, Young People and their Families is a newly registered service with the Care Inspectorate Scotland which offers the following: -

  • The provision of responsive and flexible support to children, young people and their families within their own home and the community, which promotes positive parenting and enhances outcomes for children and young people in their care on a social, emotional and practical level.
  • To provide practical, emotional and social support to children and young people with Additional Support Needs in their own home aimed at preventing social isolation, improving school attendance and relationships with their family and peers.
  • To assist children, young people and their families access additional social support and resources in their community ranging from Voluntary Sector organisations, Leisure facilities, training, and employment opportunities.
  • To provide emergency respite, place of safety and short breaks for children, young people and their families as and when needed to enhance the types of crisis support, intervention and planned support we can offer as part of our new service.

A key focus of this service is supporting children and young people with disabilities, additional support needs, and complex care requirements. We aim to provide consistent, relationship-based support that promotes inclusion, independence, and improved quality of life for both the young person and their family.

Our approach centres on early intervention, trauma-informed practice, and tailored support, helping families build resilience and achieve positive, sustainable outcomes.

The Role

Reporting to our Registered Services Manager, you will work closely with children, young people and their families in their homes and local communities, delivering bespoke support packages.

This role requires strong experience and confidence in supporting children with physical disabilities, learning disabilities, autism, and other additional support needs. You will play a vital role in enhancing daily living skills, promoting engagement, and supporting emotional wellbeing.

Key Responsibilities

  • Provide 1:1 and family-based support to children and young people, with a strong focus on those with disabilities and those with additional support needs.
  • Deliver practical, emotional, and behavioural support in their own home and community settings
  • Support personal care, mobility, communication needs, and daily routines where required
  • Promote independence, inclusion, and participation in social, educational, and recreational activities
  • Work collaboratively with families to identify strengths and set realistic, meaningful goals
  • Support emotional regulation, structure, and routine, adapting approaches to individual needs
  • Liaise with professionals including social work, education, health, and third-sector services
  • Maintain accurate records and contribute to care and support planning
  • Uphold safeguarding, wellbeing, and best practice standards at all times

About You

We are seeking skilled, compassionate, and experienced individuals who are passionate about supporting children with disabilities and additional support needs, making a lasting difference to children, young people and their families.

Essential:

  • Significant experience working with children and young people with disabilities, additional support needs, or complex care requirements
  • Experience supporting autism, learning disabilities, physical disabilities, or neurodiverse needs
  • Ability to deliver person-centred, trauma-informed care
  • Strong communication, patience, and relationship-building skills
  • Confidence working in family homes and community settings
  • Flexibility to work evenings and weekends where required

Desirable:

  • Relevant qualifications in Health & Social Care, Social Care, Childhood Practice, or a related field e.g HNC Health and Social Care, SVQ 2 or 3 in children and young people
  • Experience in care, early intervention, or family support services
  • Knowledge of GIRFEC, safeguarding practices, and positive behavioural support (PBS)
  • Experience with supporting people in crisis situations and those with complex care and support needs.
  • Full UK driving licence and access to a vehicle
