Weare currently seeking a Health Care Assistant to work on a Bank Contract in ourLymphoedema Clinic.
TheLymphoedema Service at Teesside Hospice provides care and treatment across TeesValley and North Yorkshire. It is a practitioner lead service providingassessment, education and treatment to patients with primary and secondarylymphoedema.
Theservice is provided from clinics, patients homes, care homes and GPsurgeries. We work closely withcommunity staff.
Experiencein lymphoedema is not essential but commitment and willingness to completerelevant training and work well in a team providing a high standard ofindividualised care is required.
Main duties of the job
To work as aneffective part of the lymphoedema team assisting in the provision of a highlevel of specialist lymphoedema care under the direction of a registerednurse.
The care providedwill be in accordance with the values set out by the hospice thus ensuringthat we can achieve the vision and mission of Teesside Hospice.

Job responsibilities
Tomaintain patient confidentiality and abide by Information Governance Standardsat all times.
Tomaintain a healthy and safe environment for all patients, staff and visitorsand demonstrate awareness of risk management and infection prevention andcontrol.
Toparticipate in agreed programmes of care ensuring that all care given isclearly and accurately documented in line within standards and seekingsupport from senior staff as necessary.
Followingtraining and assessment of competence to educate the patient on how to self-managelymphoedema.
Toteach the patient on the cornerstones of lymphoedema management, includingskin care, exercise, fitting of compression hosiery and self-lymphaticdrainage. Record limb volumes.
To manage your workload/tasks as designatedby the registered nurse leading the team.
Toactively participate in clinical audit.
Toensure the maintenance of confidentiality including accurate and timelyrecords.
Toliaise with and provide relevant information to other members of the multidisciplinary team, both internallyand externally.
Tomonitor stock levels and order products as required.
Tohave an awareness of costs of supplies and clinic budget.
Withdirection/supervision from Lymphoedema Practitioner, following a care plan once competencies completed, perform higher level of skills:
* Kinesio Tape
* Intermittent Pneumatic Compression Therapy
* Knowledge of Cellulitis and its management
* Understand contraindications in Lymphoedema management, to be aware of red flags and when to raise concerns with a Lymphoedema Practitioner
* Understand how Lymphoedema management can be modified in Palliative Care
* Under direction of the Lymphoedema Practitioner and following the care plan, fit compression garments in the patient’s home/ Care Home following the Lone worker Policy
Toadhere to Teesside Hospice policies, procedures, guidelines and standards atall times and to promote these to others.
Adopt the role of link worker / linkpractitioner as required by the hospice management team.
Attend and participate in an annualappraisal and regular reviews of performance and objectives, identifying gapsin knowledge/skills and developing a plan to enhance service andself-development.
To encourage and promote thephilosophy of Teesside Hospice and present a good public image, to co-operatewith all staff in maintaining good relationships with outside agencies inorder to uphold the charitys image and to win increased support for its work.
To be able to communicate effectivelywith all members of the team and to develop excellent relationships withcommunity teams as appropriate to the role.
To participate in mandatory Clinicalsupervision or reflective practice as necessary as directed by the clinicalleader.
To attend staff meetings andparticipate in other meetings as appropriate.
To undertake risk assessment andmanagement to ensure that staff, families, and visitors are safe.
To be aware of the adult and childprotection procedures and policy within Teesside Hospice, to attend allsafeguarding training and to adhere to the Local Safeguarding Procedures atall times.
To participate in the induction ofnew staff to the department and wider hospice as necessary.
To support student nurses duringtheir placement at Teesside Hospice and work alongside them as directed bythe registered nurse.
To be able to carry out effectivecomplaint, accident and incident reporting.
To play an active part in the Clinical Governance framework within the hospice.
To be aware of the InfectionPrevention and Control policies and work in accordance with IPC procedures atall times.
To be aware of, and responsive to,the changing nature of the Hospice and adopt a flexible and pro-activeapproach to work.
To undertake administration changesin accordance with hospice policy

Education& Training
Attend all mandatory training coursesi.e. Resuscitation, Moving and Handling, and Health and Safety training.
Access THCF in-house trainingprogramme to further develop your knowledge.
Attend and complete relevant LymphoedemaTraining, including Lymphoedema Theory and Practice.
Attend and complete clinical skillstraining relevant to Lymphoedema and chronic illness.
Maintain own Portfolio, and complete corecompetencies.
Person Specification
Qualifications
* Minimum level NVQ level 2 or 3 in Health & Social Care.
* A good standard of general education.
Experience
* Some experience of working with patients with life limiting illness or end of life care.
* Understanding of and adherence to patient confidentiality
* An understanding of what a person-centred approach.
* Understanding of safeguarding and preventing patients from abuse.
* Understanding of infection prevention and control.
Other
* Full UK Driving licence and access to a car for work purposes
