We are seeking an experienced registered nurse to join our District Nursing Team in Tywyn. The role is dynamic, proactive, and progressive, with commitment to making a difference for patients.
Career Pathways
Highly motivated and passionate Band 5 Community Nurses are encouraged to develop towards one of the following roles:
* Band 6 District Nurse by completing the District Nurse Qualification.
* Chronic Condition Case Manager by undertaking disease‑specific training.
* Nurse Practitioner by completing a modular programme to attain required clinical skills.
Main Duties
* Provide clinically effective care within the District Nurse Team and deliver evidence‑based care across Anglesey and Gwynedd.
* Travel between patient homes, in towns and rural areas, in a timely manner.
* Communicate effectively and maintain high care standards.
* Possess clinical competency in catheter, PICC line, wound, leg ulcer care and experience in palliative care.
* Competent in CRT working.
* Work unsociable hours and be able to travel between patient homes.

Qualifications
* Registered nurse or within 2 months of gaining PIN.
* Knowledge of community care.
* Practice supervisor or assessor.
Desirable Qualifications
* Previous community experience.
Skills
Essential Criteria
* Effective interpersonal and communication skills.
* Ability to maintain high standards of care.
* Clinical competency.
* Competent in catheter care, PICC line care, wound care, leg ulcer care, and experience in palliative care.
* CRT working.
Desirable Criteria
* Flexible approach to work.
Knowledge
Essential Criteria
* Evidence of ongoing professional training.
* Knowledge of professional codes of conduct.
