Responsibilities
* Join a Social Work Community Team to develop your practice across an all‑age resident population.
* Enable effective integrated working with partners and promote independence through reablement and enablement approaches.
* Safeguard vulnerable people and undertake Section 42 Enquiries.
* Promote the principles of choice and control, personalisation and self‑directed support.
* Collaborate with other Social Workers, Occupational Therapists and Community Care Officers in a fast‑paced team.
Qualifications
* Social Work Qualification and registration by Social Work England.
* Additional qualifications accepted: Approved Mental Health Practitioner (AMHP), Best Interests Assessor (BIA) or Practice Educator (Level 2).
* Experience in all areas of Social Work practice and hospital discharge.
