Imperial College Healthcare NHS Trust (ICHT) is recruiting an experienced Band 6 Registered Nurse to join our Virtual Ward Team, supporting the delivery of high quality, digitally enabled care in line with the NHS Long Term Plan.
Virtual Wards play a key role in providing acute level care to patients in their own homes through patient onboarding, digital platforms, and multidisciplinary working. The post holder will be responsible for onboarding and/or remote monitoring of patients on virtual ward pathways and will support the operational delivery of the Virtual Wards service across the Trust.
The role offers an opportunity to contribute to an innovative model of care, develop advanced clinical and digital skills, and support service development across ICHT.
Main duties of the job
1. Undertaking onboarding, clinical assessment, monitoring, and management of patients on digital virtual ward pathways.
2. Reviewing and processing referrals, assessing appropriateness, and leading the onboarding of patients onto virtual ward pathways.
3. Ensure all patients receive safe, efficient, and high quality care in accordance with Trust policies and professional standards.
4. Maintain accurate and timely clinical documentation to support effective monitoring and escalation of care.
5. Support the Virtual ward Matron, Clinical Lead, and Heads of Pathway in providing clinical leadership across virtual ward pathways.
6. Contribute to education, engagement, and awareness of the Virtual Wards Programme among staff and patients.
7. Support the development of digital personal health records and assist hospital teams in achieving timely patient review, transfer, or discharge.
8. Participate in service improvement initiatives and contribute to the ongoing development of the Virtual Wards model.
