Contract: Temporary, Full-time
Responsibilities:
1. Coordinate safe and timely discharges from hospital
2. Assess the needs of patients and their families
3. Develop and implement care plans
4. Liaise with health professionals and community services
5. Provide support and advice to patients and their families
Requirements:
6. Degree in Social Work
7. Registered with the Scottish Social Services Council (SSSC)
8. Experience in hospital discharge planning
9. Knowledge of relevant legislation and policies
10. Excellent communication and interpersonal skills
