Job Purpose:
We are seeking a highly skilled and passionate Community Occupational Therapist to join the team. The successful candidate will provide specialist occupational therapy assessments and interventions for individuals in the community setting, applying the Model of Human Occupation (MOHO) framework. This role is crucial in supporting patients to regain independence, improve their quality of life, and participate actively in their communities.
Key Responsibilities:
Specialist OT Assessments & Interventions:
Deliver comprehensive assessments and tailor interventions that focus on enhancing occupational performance and improving quality of life. Utilise the Model of Human Occupation (MOHO) framework to guide clinical decision-making.
Individual & Group Interventions:
Provide a range of one-to-one interventions and group-based activities aimed at promoting recovery, occupational participation, and community integration.
Innovative Group Programmes:
Contribute to the development and delivery of creative group programmes that focus on increasing occupational participation and supporting individuals in their transition to community-based living.
Holistic Approach to Care:
Work closely with multidisciplinary teams to ensure that all aspects of the individuals physical, mental, and emotional well-being are considered when planning care and interventions.
Client-Centered Practice:
Engage with clients in a collaborative manner, ensuring that their personal goals, preferences, and needs are prioritised in the development of treatment plans.
Professional Development & Training:
Engage in continuous professional development opportunities and stay up to date with the latest evidence-based practices in occupational therapy.
Essential Qualifications & Experience:
Degree in Occupational Therapy or equivalent.
HCPC (Health and Care Professions Council) registration as an Occupational Therapist.
Proven experience working in a community-based setting, with a focus on adult mental health or rehabilitation.
Strong understanding and experience in applying the Model of Human Occupation (MOHO) framework.
Experience in delivering both individual and group-based interventions.
