We are looking for a dynamic Registered Nurse leader who is motivated, forward thinking and has clinical experience in palliative and end of life care to lead our Community Palliative Care team in the East Somerset area. In addition to leading a responsive team, the post holder will need to be able to flex to the needs of our community, and seek innovative improvements in community based MDT working in collaboration with System partners.
CLINICAL LEAD - COMMUNITY PALLIATIVE CARE | Band 7 £44,617 - £51,057 per annum
As the Clinical Lead for the Community Palliative Care team, you will:
* Contribute to the delivery of the shared vision and strategy of the Patient and Family Services Directorate in order to deliver the Everyday Everyone Strategy and associated key Organisation Objectives.
* Lead and inspire a culture of continuous quality improvement and service development of new models of Community Palliative Care Teams.
* Demonstrate leadership of the organisational values and enable palliative care teams to embed a culture of work that reflects these values.
* Coach teams to develop matrix approach both internally within Dorothy House services and externally with other partner organisations and groups within the community to maximise service provision for people with end of life conditions.
* Lead and support teams to develop strong working relationships with partnership organisations to enhance knowledge and practice in palliative care for Care Homes and other areas.
* Provide nursing leadership in specialist palliative care through advice, support and education to the Community Palliative Care Teams (CPCT) and other health professionals.
* Manage a limited caseload providing specialist palliative care to those with a life threatening illness and their families in partnership with health and social care colleagues

HOURS OF WORK
This is a Full Time position working 37.5 hours over 5 Days, Monday to Friday.
ABOUT YOU
Due to the community based nature of the role, the post holder will need to hold a full UK driving licence with access to a vehicle.
Essential requirements:
* Registered Nurse
* Postgraduate or equivalent level of knowledge in a relevant subject
* Teaching and assessing qualification
* Independent Prescriber or working towards qualification
* Clinical experience in specialist palliative care
* Management/ leadership skills experience
* Clinical assessment and analytical skills
* Self-motivated and able to work as part of a team including the Multi-disciplinary Team
* Ability to analyse data and research in practice
* Excellent communication and interpersonal skills
* Confident in use of IT systems including patient systems
* Car driver essential to support the ability to deliver patient services across a geographical patch
Desirable requirements:
* Community qualification
* Relevant community experience
