Overview
We have an exciting opportunity for a band 5 newly qualified social worker (NQSW) to join the Lichfield community team. This is an ideal opportunity for a final year social work student to join a supportive, responsive, nurturing team which invests in training and professional development. This includes access to the Assessed and Supported Year in Employment (ASYE) programme. The successful applicant will deliver holistic social care assessments, reviews and case management in partnership with service users, families, carers and other professionals to achieve the best outcomes for the person. The team is community based and works with adults over the age of 18 with a physical disability and older adults over 65 living with physical disabilities and/or a mental health dementia diagnosis. This is a full-time position (37.5hrs per week). The team is based in Lichfield and the role will involve travel as required. The post holder will need to have a full driving licence and access to a car.
Responsibilities
* Deliver holistic social care assessments, reviews and case management in partnership with service users, families, carers and other professionals to achieve the best outcomes for the person.
* Work with adults in community settings to support independence and wellbeing.
* Collaborate with multidisciplinary colleagues and carers to develop and implement care plans.
* Coordinate care activities and monitor progress, adjusting plans as needed.
Qualifications and requirements
* Band 5 Newly Qualified Social Worker (NQSW) or final year social work student eligible for ASYE
* Auto-enrolled in the SWLA and aligned to the BASW Professional Capabilities Framework stages (ASYE, Social Worker or Advanced Social Worker) based on prior CPD and experience
* Willingness to take responsibility for ongoing CPD
* Full driving licence and access to a car; must be able to travel as required
* Awareness of AI usage considerations in application processes (if applicable)
