Early Intervention in Psychosis Team Coventry – Care Coordinator
Band 6. Full time 37.5 hours. This role is open to a Nurse, Occupational Therapist or Social Worker to join our well‑established Early Intervention team as a Care Coordinator. The team works with individuals experiencing the first onset of Psychosis.
The role involves a detailed initial assessment incorporating the CAARMs assessment to identify at risk mental states (ARMS) or first episode psychosis (FEP) and incorporating it into a formulation to plan the client's care over the next three years.
The team uses a Recovery philosophy to collaboratively identify the individual's goals and needs and to support them towards achieving these. The successful applicant will be expected to work creatively both with other team members and independently to support the client achieve their goals, such as in housing, finances, vocational activities and social inclusion.
Responsibilities
* Conduct detailed initial assessment using the CAARMs assessment to identify ARMS or FEP.
* Develop a formulation and care plan for the client over the next three years.
* Collaboratively identify goals and needs using a Recovery philosophy.
* Support clients in achieving goals related to housing, finances, vocational activities and social inclusion.
* Work creatively with team members and independently to help clients become independent and reduce ongoing mental health service input.
