Hospital Social Work Team – Lincoln County Hospital
Position: 1 x Part‑Time (23 hours per week) Permanent Position.
Role Overview
We are looking for enthusiastic and motivated Level 1 Social Workers to join our Hospital Social Work Team. The role supports patients across Lincoln County Hospital and other hospitals in Northern Lincolnshire, working with health colleagues to ensure safe, person‑centred discharges.
Responsibilities
- Manage a caseload under appropriate supervision, developing professional confidence and autonomy.
- Undertake Care Act 2014 assessments, care and support planning, and reviews.
- Work collaboratively with individuals, families, carers and professionals to identify strengths and promote independence.
- Contribute to safeguarding enquiries, ensuring the person’s voice and desired outcomes remain central.
- Support safe and timely hospital discharges in partnership with health colleagues.
- Identify wider needs such as housing, health or community support and work with partners to address them.
- Promote strengths‑based practice and help people build and maintain support networks.
Qualifications and Experience
- You are a Level 1 Social Worker, newly qualified or early career.
- You are registered with the relevant professional body.
- Experience or interest in Care Act practice, strengths‑based assessment and multidisciplinary working.
- Strong communication skills, able to use accurate spoken English (Fluency Duty).

Working Pattern
Standard working hours: Monday – Thursday 8:45am – 5:15pm, Friday 8:45am – 4:45pm. The service also operates a weekend rota – one weekend in four: 8:30am – 4:30pm. When you work a weekend, you receive a day off during the week.
