This role is an in person role working at Bow Medical Practice and Wallingbrook Health Group.


Job summary

Undertake clinical medicationreviews with patients and produce recommendations for Nurses and/or GP onprescribing and monitoring. See (where appropriate) patients with single ormultiple medical problems where medicine optimisation is required Review theon-going need for each medicine, a review of monitoring needs and anopportunity to support patients with their medicines taking ensuring they getthe best use of their medicines. Make appropriate recommendations to GPs formedicine improvement. Risk stratification: Identification of cohorts ofpatients at high risk of harm from medicines through pre-prepared practicecomputer searches. This might include risks that are patient related, medicinerelated, or both. Unplanned hospital admissions. Review the use of medicinesmost associated with unplanned hospital admissions and readmissions throughaudit and individual patient reviews. Put in place changes to reduce theprescribing of these medicines to high-risk patient groups. Provide medicinessupport for patients with questions, queries and concerns about their medicines.

To reconcile medicines following discharge from hospitals,intermediate care and into Care Homes, including identifying and rectifyingunexplained changes and working with patients and Community Pharmacists toensure patients receive the medicines they need post discharge.


Main duties of the job

The post holder will work within their clinical competencies as part of a multi-disciplinary team to provideexpertise in clinical medicines management, structured medication reviews,manage long term conditions, management of medicines on transfer of care andsystems for safer prescribing and support acute prescription request, whileaddressing both the public health and social care needs of patients within allof the GP Practices within the PCN.

The post holder will performface to face and telephone medication review of patients with polypharmacyespecially for older people, and those with multiple comorbidities.

The post holder will provide leadership on qualityimprovement and clinical audit and well as managing some aspects of the Qualityand Outcomes Framework. This role is pivotal to improving the quality of careand operational efficiencies so requires motivation and passion to deliver anexcellent service within General Practice. The post holder will be supported todevelop their role to become a non-medical prescriber, if that qualification isnot already held.

This role will be based at 2 practices in the PCN, these are Bow Medical Practice and Wallingbrook Health Group. The role includes workingwith and alongside, The PCN Pharmacy team, Prescription Teams and Dispensaryteams at all of the practices within the PCN.


Job responsibilities

Management of medicines at the transfer of care. To reconcile medicinesfollowing discharge from hospitals, intermediate care and into Care Homes,including identifying and rectifying unexplained changes and working with patientsand Community Pharmacists to ensure patients receive the medicines they needpost discharge. Set up and manage systems to ensure continuity of medicinessupply to high-risk groups of patients (e.g. those with medicine complianceaids)

Medicine information to Practice staff and patients. Answer relevantmedicine-related enquiries from GPs, the wider MDT team, other network staffand Healthcare Teams.

Support andwork alongside the Pharmacy Technician, Prescription Team and Dispensary Teamsupervising and advising when appropriate. Work alongside the extended PCNPharmacy Team to deliver the PCN shared agenda.

PROMOTE PRESCRIBING QUALITY AND SAFETY INCLUDING:

Support repeat prescribing policy within each Practice.

Manage the repeat prescribing reauthorisation process by reviewingpatient requests for repeat prescriptions and reviewing medicines reachingreview dates and flagging up those needing a review. Ensure patients haveappropriate monitoring tests in place when required.

Analyse, interpretand present medicines data to highlight issues and risks to support decisionmaking. Undertake clinical audits of prescribing in areas directed by the GPs, feedbackthe results and implement changes in conjunction with the relevant practiceteam. Implement changes to medicines that result from MHRA alerts, productwithdrawal and other local and national guidance.

Implementation of local and national guidelines andformulary recommendations. Monitor Practice prescribing against the localhealth economy's RAG list and make recommendations to GPs for medicines thatshould be prescribed by Hospital Doctors (red drugs) or subject to shared care(amber drugs).

Liaise with colleagues including ICB Pharmacists and Pharmacy Technicians Headsof Medicines Management/Optimisation to benefit from peer support. Foster andmaintain strong links with all services across the PCN and neighbouringnetworks. Explore the potential for collaborative working and takeopportunities to initiate and sustain such relationships. Liaise with otherstakeholders as needed for the collective benefit of patients, including butnot limited to: Patients and their representatives GP, Nurses and otherPractice staff Social Prescribers, First Contact Physiotherapists, Physicians,Associates and Paramedics and hospital staff.


Person Specification


Qualifications

* Maintain registration with the General Pharmaceutical Council
* Relevant undergraduate degree in pharmacy (BPharm, MPharm or equivalent)
* Independent prescriber or working towards/ intent of gaining qualification
* Minimum of 5 years post registration experience
* Portfolio of evidence of continuous professional development and post qualifying experience
* Able to drive and has access to car or equivalent
* Knowledge/experience of EMIS Web and SystemOne
