Dixton Surgery is a busy, dispensing practicesituated on the edge of Monmouth. We are a border practice, so have patientsand secondary care providers in both Wales and England. We are a rural practiceso have a large practice area covering a large part of the beautiful northMonmouthshire countryside entering into Herefordshire and Gloucestershire alongthe River Wye.Our practice has a growing population of over 5,000 patients, with aninteresting mix of town and country, the young and old. The Practice shares the clinical leadershipand medical staffing of the local hospital (Monnow Vale Community Hospital)where we provide inpatient care for about five patients at a time. We serve three large Care Homes and also alarge Forensic Psychiatric Hospital.The Practice is currently run by 4 GP Partners and 1 Management Partner. We have over 20 team members covering nursing, admin, reception and dispensary.
Job responsibilities
Accurately process, dispense and check prescriptions following practice policies and procedures.
Support the GP's and the wider clinical team to undertake structured medication reviews
Answer medication related queries from patients and health care colleagues
Processing hospital discharge summaries and liaising with patients, GPs and community pharmacies to ensure any medicine changes and necessary reviews are carried out
Advising on the monitoring of high-risk drugs, and ensuring patients attend for appropriate routine monitoring
Liaising with community pharmacies and care homes about out of stock medications to ensure good patient care
Carrying out regular audits of prescribing safety using the GP practice clinical system
Contribute to financial monitoring and cost improvement strategies
Contribute to the development of a robust process to reclaim costs associated with dispensary and medication purchases.
Acting upon MHRA drug alerts and recalls and communicating any outcomes with appropriate colleagues as necessary
Communicating with patients to ensure effective medicine use
Person Specification
Qualifications
* NVQ/BTEC level 3 in pharmacy services
* Flexible to meet the needs of the practice
* Excellent communication and organisation skills
* Ability to work under pressure and meet deadlines
Experience
* Experience in primary care
