<div><p><b>DNSPQ STUDENTS ARE WELCOME TO APPLY</b></p><h3>Job Overview</h3><p>The Caseload Holder will manage a defined caseload in line with good practice during a 24‑hour period. The post holder will be involved in complex needs assessment (CHC, health and social care) and the provision of specialist district nursing practice, work collaboratively with other agencies for patient services without direct supervision, undertake management duties, and deputise in the absence of the Team Leader.</p><h3>Main Duties</h3><ul><li>Responsible to the Team Leader and accountable for a designated caseload of patients.</li><li>Lead a proactive, multi‑skilled district nursing team committed to preventing hospital admissions and ensuring delivery of nursing excellence.</li><li>Apply strong leadership and organisational skills in busy, pressured environments while maintaining a high standard of nursing care.</li><li>Maintain evidence of continued professional development.</li><li>Speak Welsh desirable; Welsh and/or English speakers welcomed.</li></ul><h3>Working for our Organisation</h3><p>Betsi Cadwaladr University Health Board provides a full range of primary, community, mental health, acute and elective services for a population of around 700,000 across North Wales.</p><h3>Person Specification</h3><h3>Essential Criteria</h3><ul><li>NMC RN first‑level nurse</li><li>DN Specialist practice qualification / certificate, diploma, degree or working towards</li><li>Nurse prescribing qualification or working towards</li><li>Evidence of ongoing education and professional development</li><li>Mentorship and assessment training</li><li>Community experience as band 5 or equivalent</li><li>Experience of working with patients with acute/complex needs</li><li>Varied clinical experience and multidisciplinary working</li><li>Analytical skills and ability to interpret clinical signs</li><li>Organisational skills</li><li>Ability to assess, plan, implement, evaluate and delegate care</li><li>Supervise, teach and assess registered and non‑registered staff</li><li>Provide mentorship</li><li>Ability to work collaboratively with primary health care team and other agencies</li><li>IT skills</li><li>Awareness of current research and its application to practice</li><li>Knowledge and evidence of application of consent</li><li>Knowledge of data protection and patient confidentiality</li><li>Understanding of risk assessment and health & safety issues</li><li>Awareness of the changing health agenda which affects community practice</li><li>Demonstrate ability to care and communicate with warmth and understanding</li><li>Excellent verbal and written communication skills</li><li>Committed to continuous development</li><li>Flexible approach to the needs of the service</li><li>Demonstrates timely travel arrangements between patients homes, clinics and surgeries</li></ul><h3>Desirable Criteria</h3><ul><li>Management qualification e.g. ILM, RCN Leadership</li><li>Link nurse role experience</li><li>Experience of undertaking audits</li><li>Recognised qualification in chronic conditions management</li><li>Ability to speak Welsh</li><li>Awareness of wider health agenda</li></ul><p>Committed to promoting equality and diversity, and a disability confident employer.
