The Department of Medicine for Older People wishes to appoint a Consultant Geriatrician with Specialist Interest in Re-ablement Medicine, based at Broadgreen Hospital. The Department of Medicine for Older People and Stroke (DMOPS) is a well-established department delivering specialised services for older people and developing evidence-based models of care delivery. The post involves sessional work on general geriatric wards and re-ablement wards, leading MDT-based CGA care on the ward, and providing medical support within the re-ablement units.
The inpatient wards comprise 23 beds with on average 2-4 new admissions each day. The post will be supported by trainees/junior doctors and Advanced Nurse Practitioners at the Broadgreen and Royal sites, with STR support on a sessional basis. The reablement wards operate with a proactive approach, in partnership with therapy and nursing for primary care and consultant support, and have been developed with ECIST support as a national example of good practice.
Responsibilities
* Provide medical leadership and responsibility for delivering comprehensive geriatric assessment (CGA) on general and re-ablement wards.
* Lead MDT-based CGA care on the ward and contribute to discharge planning for complex patients requiring re-ablement.
* Work sessionally on inpatient wards (23 beds) and two 23-bed reablement wards, with support from a multidisciplinary team and ACP where applicable.
* Collaborate with trainees, junior doctors, Advanced Nurse Practitioners, and STR to deliver high-quality inpatient care.
Requirements / Qualifications
* Consultant Geriatrician with Specialist Interest in Re-ablement Medicine.
* Experience in geriatric medicine with a focus on re-ablement, rehabilitation, and complex medical optimization.
* Ability to lead MDT-based CGA and contribute to discharge planning.
