The Role The service provides specialist palliative care nursing across Lanarkshire which has a population of approximately 655,000 people, 92 GP practices, 3 community hospitals and numerous nursing / care homes. Referrals are made for patients who have complex care needs requiring the support of a specialist palliative care clinical nurse specialist, working as part of a multidisciplinary team The Advanced clinical nurse specialist (ACNS)- specialist palliative care (SPC) is an experienced and highly educated registered nurse working within this specific field who is able to demonstrate competence within all four pillars of practice. Educated to Masters level (at least post graduate diploma in palliative care or cancer or equivalent prior learning and clinical experience) and understands the constructs of specialist palliative care nursing in community and acute settings. The practitioner will work autonomously across all settings as an integral part of the pan Lanarkshire specialist palliative care service, across seven days to meet the demands of the service. The post holder will be an expert practitioner and will manage all aspects of the patients’ care, including future care planning and care around death. The ACNS will provide holistic assessment for patients with complex physical, emotional and psychosocial palliative and end of life care needs. They will analyse and synthesise clinical information based on the patient’s presentation, history, physical and psychological examination and findings from relevant investigations to make a differential diagnosis or confirm a diagnosis. Then in partnership with the patient and family, the ACNS will formulate a person centered plan, which may include prescription of medications and deliver clinical care and management of both stable and deteriorating patients. Making complex, autonomous decisions using advanced skills and knowledge, with overall responsibility for the assessment of care needs and the delivery of advanced level practice within the SPC service. Advanced Specialist knowledge and experience of relevant conditions, pathology, policies and procedures associated with the speciality/area of practice ensuring that the level of expertise can be utilised to deliver leadership within speciality. Evidence of application of advanced practice skills in speciality and applying clinical judgement. Evidence of further developed knowledge and experience in management and/or leadership. Further education/evidence of Continuous Professional Development in area of speciality including study days/courses/post graduate qualification The post holder will possess excellent team-working/leadership skills and have the ability to motivate and influence others and work using own initiative Excellent listening, communication and interpersonal skills and effective time management skills Working knowledge of basic information technology Car driver with a full, valid UK/EU/EEA drivers licence Please note for all vacancies where a driving licence is required this must be a full UK/EU/EEA licence.
