We have an exciting opportunity for an experienced Social Worker to join our Community Team for People with Learning Disability (CTPLD). This is a full‑time permanent role with hybrid working, salary £40,192‑£44,012 per annum plus an annual retention bonus of £3,000 subject to eligibility.


The Role

You will be working as a Social Worker principally to support young people in transition from Children’s to Adult Learning Disability services.


Your Role Will Involve

* Located within a multi‑disciplinary community learning disability team, you will support young people with learning disabilities and their families and carers to make a positive transfer to adult services, liaising with Children’s Services, Health Services, Education, Care Agencies and other organisations.
* You will carry out assessments, undertake care reviews, support care planning and make funding applications.
* You will take part in a duty rota to deal with incidents, safeguarding alerts and information requests.
* As a Social Worker you will protect and promote the interests of vulnerable people.
* You will undertake safeguarding investigations, Deprivation of Liberty Safeguards assessments, capacity assessments and best‑interest decisions for adults with a learning disability.


What We Are Looking For

* A qualified Social Worker with a BA, BSc Diploma or equivalent.
* Current registration with Social Work England.
* Experience working in a health and social care environment, ideally across children’s and adult services.
* Ability to work independently to tight deadlines, balancing complex and sometimes contentious work.
* Experience completing Community DOLS applications, Assessments of Need, Continuing Health Care Applications, service planning and funding applications.
* Effective resource management combined with a commitment to the best outcomes for the young person and their family.
* Good understanding of the Care Act and other relevant legislation.
* Ability to work closely with others.
