A new role has become available with the Wiltshire Dementia Complex Intervention Team (DCIT).
As a key worker in the team, you will be responsible for a caseload of service users diagnosed with dementia, working within the 'Your Team, Your Conversation, Your Plan' framework to deliver a range of health and social care interventions. You will receive excellent supervision, and there will be opportunities for career development and training to support you in this role.
This position is based in the west of the county and primarily involves working in that area. However, since the service covers Wiltshire-wide, occasional travel within the county may be required. Due to the size of Wiltshire and the nature of the role, applicants must hold a full driving license and have access to their own transport, as public transport will not suffice for the requirements of this post.
Main duties of the job
The Complex Intervention Team aims to provide safe and supportive mental health care for adults with dementia experiencing distress or difficulties related to their illness.
The team offers a range of community-based health interventions, working inclusively to resolve immediate difficulties and improve service users' situations. You will act as a care coordinator.
Where appropriate, you will collaborate with other services to plan, deliver, and review effective interventions to manage risk and prevent hospital admissions.
We welcome applications from Band 5 practitioners with relevant postgraduate experience. We will also consider Band 5/6 development posts.
