Are you a passionate and experienced advanced nurse practitioner or advanced paramedic ready to lead innovative, preventative care in the heart of our community? Clacton PCN is looking for a proactive care lead to drive forward integrated, person-centred care across our network. As a proactive care lead, you will head up a dynamic multidisciplinary team including nurses, care coordinators, paramedics, HCAs, and social prescribing link workers. You’ll be responsible for delivering high-quality, proactive, and preventative care with a particular focus on: Frailty management Cardiovascular disease prevention Enhanced Health in Care Homes (EHCH) Embedding population health management approaches You’ll work closely with PCN leadership and wider system partners to deliver care that empowers patients, reduces hospital admissions, and supports people to live well at home for longer. About the Candidate To be considered for this position, you will need the following; Registered nurse, paramedic, or AHP (with valid registration) Independent prescriber (V300) Proven leadership experience in healthcare Strong clinical background in frailty or long-term conditions Skilled in team coordination, communication, and digital tools Committed to patient-centred care and service transformation Desirable: Master's level qualification in Advanced Practice or Leadership Experience with PHM data and digital health tools Knowledge of the NHS Long Term Plan and Neighbourhood Care Programme Additional Requirements: Full UK driving licence Enhanced DBS check About Us Clacton PCN comprises six forward-thinking GP practices: East Lynne Medical Centre (Lead Practice), North Clacton Medical Group, Old Road Surgery, Thorpe Surgery, Fronks Road Surgery, and Harewood Surgery.
