ThornburyMedical Centre looking for 2 part-time experienced Practice Nurse's to join ourvibrant and friendly Practice andnursingteam.

We are looking for aforward thinking experienced Practice Nurse who has the qualifications and/orexperience to support our patients with long term health conditions and becomfortable with the ever-changing day to day world of General Practice.
Main duties of the job
Experienced in Primary Care, the applicant can expect to be involved in Chronic Disease Management (Diabetes, CVD, Respiratory Disease and of course Long Covid etc), otherQuality and Improvement Goals Learning Disability Health Checks and MentalHealth Physical Health Checks, Vaccinations and Immunisations (Adult andChild), Cervical Screening, Spirometry, ECGs, Contraception and Sexual health,Travel Medicine, Ear Irrigation and Venepuncture.
Good Knowledge of Clinical Governance and Infection Prevention Controlprinciples are essential. Good standard of written English and computerliteracy are required.
On occasion it may be necessary to undertake home visits ( fluvaccination and long term disease monitoring).

We are anaverage sized practice with just over 7,000 patients. GPs, Advanced Clinical Practitioner,Pharmacist, HCA and Physician Associate and other health professionals.
We have anadmin and reception team that supports the clinical team and patients.
We are partof Bradford PCN 5 Primary Care Network and work closely with our neighbouring practices.

Job responsibilities
The post holder is responsible for the delivery of practicenursing services and care to the practice population. Supported by GPs,Practice Manager and the Lead Practice Nurse, you will deliver care within yourscope, focus on supporting patients to be healthy, monitoring long-termconditions, health prevention and screening activities. You will workcollaboratively with the whole team to meet the needs of patients and improveoutcomes, supporting the delivery of policy and procedures and provide nurseleadership as required.
The post holder will also strive to maximise the practices achievementof both Local and National Quality and Performance objectives.

Qualifications
Essential
1. Registered Nurse with relevant Nurse Qualification / Degree.
2. Post graduate education in the management of Diabetes, Asthma and COPD.
Desirable
3. Current (within the last 12 months) emergency resuscitation training certificate.
4. Experience of Systmone Clinical System.
Experience

5. Car driver with clean licence.
6. Able to work at the desired times.
7. Flexible and able to cover for colleague absence through sickness, annual leave or study leave.
8. Experience of primary care.
9. Experience and competent in General Treatment Room Duties. Experience of working in accordance with the NMC Code of Practice. Experience of dealing with the public/patients
10. Ability to work without direct supervision and determine own workload priorities.
11. Ability to work as part of an integrated multi-skilled team.
12. Able to work under pressure.
13. Ability to use own judgement, resourcefulness and common sense, willingness and enthusiasm to developing nursing skills
