Job Overview
We are seeking an enthusiastic Clinical Lead to join the Chester South Community Care Team. You will be part of a multidisciplinary team including a team manager, enhanced clinical practitioner, clinical leads, community nurses, AHPs, and other specialists. The ideal candidate will be confident in providing leadership, support, and coaching to clients and clinicians, with excellent communication skills and the ability to work in partnership with clients, families, and carers, safeguarding individuals at risk.
Main Responsibilities
1. Provide clinical oversight and leadership for a designated caseload.
2. Collaborate with health, social care, and third-sector partners to meet patient needs.
3. Ensure patients receive appropriate care promptly and efficiently.
4. Focus on person-centered care, reducing delays, and promoting independence.
5. Align care delivery with local health and social care agendas and initiatives.
6. Understand local public health profiles to inform care planning.
7. Manage caseloads effectively, monitoring standards and quality of care.
8. Provide clinical leadership, coaching, and development to the team.

Minimum Requirements
* First level registered general adult nurse
* Evidence of teaching and assessing skills
* SPC qualification in District Nursing or equivalent, with demonstrated caseload management and health needs assessment experience
* Proficiency in evidence-based care delivery
* Significant community healthcare experience and ability to make autonomous decisions
