We are looking for a registered nurse interested in Haemoglobinopathies (Sickle Cell & Thalassemia) to join our Haematology teams cross site. As part of the sickle cell improvement programme, we are wanting to recruit an empathetic individual to join our teams, working with the sickle cell and thalassemia population across Bedfordshire.
The post holder will be responsible for the ongoing development and delivery of a high-quality sickle cell and thalassemia service across the trust. They will provide nursing leadership in coordination of care across inpatient, emergency and day case services.
This is an opportunity to demonstrate clinical leadership and practice at an advanced level, demonstrating autonomy and clinical decision making. You will work as a highly specialised member of the Haematology team to assist in enabling the patients to live as a productive member of society and reducing protracted inpatients stays.
Main duties of the job
1. Working with an experienced team of specialist doctors and nurses across Bedfordshire to deliver and develop a high quality, patient focussed Haemoglobinopathy service
2. Provide care and support to individuals and families with sickle cell disease and thalassaemia
3. Contribute and work in partnership with the HCC (Haemoglobinopathy Coordinating Centre) network, working groups and other colleagues to enhance delivery of care
4. Provide education to service users and their families regarding their disease
5. Provide teaching and training on Haemoglobinopathies to the wider team and trust
6. Demonstrate knowledge on Haemoglobinopathies and be willing to learn or expand on expertise in this area
7. Support the ongoing development of the apheresis project, working in collaboration with the multidisciplinary team.
8. Once the apheresis project is set up, ensure nursing related pathways, protocols and training is in place, leading to provision of a gold standard service.
9. Communicate effectively with patients and colleagues ensuring an individualised and equitable approach to all
10. Act as a key worker to those patients with Haemoglobinopathies
11. Support and assist in the implementation of nurse led clinics, monitoring patients with Haemoglobinpathies appropriately

Detailed job description and main responsibilities
Clinical
·Work in partnership with service users and families to develop, review and implement programmes of care, delivering advice to support health promotion, improved quality of life and prevent illness
·Utilise specialist skills during assessment and interpretation of acute and chronic conditions, escalating when appropriate
·Communicate effectively with the wider team and patients, including sharing ideas, information and feedback
·Promote health through empowering patients and their carers by offering appropriate education and advice to enable patients to make informed decisions about their care
·Demonstrate an understanding of Haemoglobinopathies, the implications of living with a chronic illness and its impact on individuals, families and the wider community
·Support and lead nurse led clinics, telephone and face to face, interpreting results, taking appropriate action and feeding back in MDT meetings and reviews
·Support the transition of adolescents to the adult services within the trust
·To be responsible for the assessment, escalation and management of risk in accordance with the trusts risk management policy
·Assist in the organisation of treatment plans ensuring patients are treated within a timely manner
Managerial/Leadership
·Contribute to the development of Haemoglobinopathy protocols and care plans
·Work collaboratively with the Haematology team to establish clear roles and responsibilities for managing of the MDT, audits and ward rounds
·Keep informed of national and Trust policies, especially those affecting service delivery and patient care
·Promote a culture that supports patient centred care and patient empowerment
·Act as role model demonstrating high standards of care and providing clinical leadership to others
·Actively participate in MDTs, acting as the patient advocate and representing nursing and patient views
