QUALIFICATIONS:
The Post holder is required to have a Registered Nurse Qualification, an appropriate recordable post basic Certificate/Statement of Competence, or equivalent level of skill acquired through experience.
The Post holder is preferred to have a minimum of one year post qualification of basic experience or an interest in Palliative Care from qualification. Consideration will give to applicants who do not fulfil these criteria based on individual employment history, experience and competence.
The selected person should have validated completion of St. Lukes Qualified Nurse Competencies and Syringe Driver Competency and for those newly employed on the Hospice Scale 14 the St. Lukes competencies will need to be achieved within the six-month induction period.
MAIN DUTIES:

RESPONSIBILITIES:
1. Clinical
1.1 Ensure the physical, spiritual, emotional, psychological, social and cultural needs of patients are identified and attended to, providing and maintaining the highest possible level of nursing care.
1.2 Devising individualised care plans and ongoing reviews to ensure care plans are individual/appropriate for patients, encouraging discussion and participation by all team members including the Multi-Disciplinary Team giving and receiving patient-centred handovers.
1.3 Active involvement within the Multi-Disciplinary Team Meetings. Ensuring patients are identified and presented for review in a clear and concise manner.
1.4 As part of patient reviews, identify and provide support to formal carers, relatives and friends of patients/residents, maintaining confidentiality at all times and ensuring good relationships with patients relatives by providing frequent contact and availability. Liaising with support services.
1.5 Ensure that all medication, treatments, therapies and care interventions are clinically reviewed and administered in accordance with Hospice Policies, Procedures and Standards.
1.6 To ensure the safe custody, storage and control and administration of all drugs in line with national standards and the Hospice Medicines Management Policy maintaining awareness of the Medicines Management meeting and role of the Accountable Officer for Medicines Management.
1.7 To work closely with all members of St. Lukes Hospice Community Services to ensure patients are supported effectively, and unwanted and inappropriate admissions to acute care are avoided.
1.8 To work effectively with patients and carers from diverse cultural and social backgrounds.
1.9 Ensure that verbal and written communication is provided to a high standard and that documentation is completed accurately on SystmOne electronic records. Sensitive information must be documented in patients paper records. To be pro-active in checking standards related to record keeping.
1.10 To ensure that effective communication is maintained between staff, patients and their relatives so that appropriate support is provided through illness and death. Maintaining coordination of patient is a key function of the St. Lukes Hospice.
1.11 Take responsibility for risk management and assessment within the work place, taking all steps possible to safeguard the health, welfare and safety of all staff, patients and visitors.
1.12 Should be providing, or working towards providing, a specialist level of practice in all areas of palliative care provision.
1.13 Ensure Hospice policies are adhered to at all time with relevant clinical policies including CVC line care, safe administration of blood products, venepuncture and cannulation.
1.14 Undertake holistic needs assessments of patients identifying physical, mental and social needs and referring as appropriate, to the Hospice and community MDTs.
2 Education/Research/Development
2.1 Attendance at In-House Study Days, Staff meetings, along with any meetings relevant to associated link role is compulsory.
2.2 Completion of organisational competencies within the first 6 months.
2.3 Participate as required in programmes of education provided by the Hospice. Staff will be required to cascade learning back to remainder of nursing staff following attendance at external Study Days.
2.4 To act as facilitator for all new members of staff, students and work experience on clinical placements as required.
2.5 To work in partnership with other teams in educational audits and triennial reviews.
2.6 To be involved in formal teaching internally and externally as required. Having an active role in promotion of hospice services i.e. Community Engagement.
2.7 Ensure the practical instruction and supervision of nurses in training allocated to the clinical area with agreed learning outcomes and current Unit teaching programmes.
2.8 To maintain awareness of relevant up to date research, in order to practice and teach research and evidence based nursing.
2.9 To maintain professional development and awareness and keep up to date with changing clinical practices. In line with Revalidation Guidance.
2.10 To actively use reflection in his/her own practice, encourage it in other team members and participate in mandatory clinical supervision.
2.11 To actively participate in team debriefs to reflect on situations and practice, cascade learning and share experiences.
2.12 To undertake extended roles in the Unit following recognised training as appropriate (e.g. Link Nurse Role). Including delivering training and ongoing support to other staff across the organisation in relation to the role.
2.13 To assist with audit activity within the Clinical areas and Hospice in general as part of the organisations annual audit plan.
3 Managerial/Organisational
3.1 To take a lead position (as the nurse in charge of the shift) as required in the day-to-day running of the Clinical Outpatients.
3.2 To demonstrate and maintain good management and organisational skills.
3.3 To ensure that both verbal and written communication is provided to a high standard and that documentation and electronic records are completed accurately and according to policy. To take a lead role in monitoring and improving these where necessary.
3.4 To assist and supervise other members of the nursing team to ensure all staff work to achieve an acceptable safe standard and high quality of care.
3.5 To report and investigate complaints/untoward incidents and complete relevant documentation.
3.6 To exercise sound leadership and clinical decision-making skills at all times and facilitate the development of same in other staff members.
3.7 To participate in the development and on-going review of Policies, Procedures and Standards as required and implementation of same within the relevant Hospice Services.
3.8 To contribute to the strategic planning for the development of staff and the service provided within the relevant Hospice services.
4 General
4.1 In the absence of Senior staff to take charge as required and liaise with Senior staff on call as necessary.
4.2 The Post holder must acquire and maintain an up-to-date working knowledge of all Hospice Strategies, Policies, Procedures and standards.
4.3 Ensure Health & Safety regulations are adhered to.
4.4 Assist in ensuring the economical use and ordering of materials and equipment, maintaining an awareness of the need for good housekeeping and helping to ensure expenditure is within budgetary limits.
4.5 To maintain strict confidentiality within the Hospice and ensure that all staff and volunteers under their management adhere to the Hospice Confidentiality Policy.
4.6 Each nurse after 6-12 months, is expected to take on a special interest or Link responsibility, in which they will be expected to act as an information resource to the rest of the team, and provide practice evidence based in that area. This will include informal and formal teaching and may include community engagement.
4.7 To undertake any other duties which are considered to be consistent and relevant to the post.
