This position is part of the Older Persons Mental Health Community Team in Basingstoke, Winchester, and Fareham/Gosport.
Responsibilities
* Support patients with dementia and their carers in their home environment through post‑diagnostic follow‑up, signposting, and carer support and education.
* Support people with functional mental health problems and their carers in the home environment during challenging times by working alongside the wider team.
* Support the team—including nurses, doctors, and psychologists—with flexible and proactive working.
* Co‑operate with the duty nurse by answering incoming calls, making referrals, and signposting to agencies.
* Review mental state competently, identifying risks and safeguarding concerns.
* Communicate effectively with the wider team.
* Accurately record service‑user information.
* Manage a caseload.
* Develop a care plan in collaboration with the patient and carer to achieve agreed goals during HCSW provision.
* Undertake baseline physical observations.
Qualifications and requirements
* Motivated, passionate, and genuinely interested in working with older people.
* Demonstrate a real commitment to providing high‑quality, compassionate care to patients and their families.
* Be a supportive team member.
* Hold a full UK driving licence and have access to a car.
Working hours: Monday – Friday, 09.00 – 17.00.
