Job Overview
The postholder will be responsible for the screening of patients and identification of care availability to facilitate safe and timely discharge from hospital. They will provide ongoing support to patients with complex needs through the Discharge to Assess model of care, working within a supportive multi-disciplinary team to promote the Home First ethos through therapeutic interventions in the patients’ own environment and on the Ready for Home wards.
Working Hours
37.5 hours per week, covering 7 days a week between 8am and 6pm.
Main Duties
* Act as a link between acute and community services, working alongside the Community Resource Team in Wrexham and Flintshire.
* Actively involved in the future development of the service.
Requirements
* Welsh language skills desirable; Welsh and/or English speakers welcome.

Person Specification
Knowledge & Qualifications (Essential)
* Registered with the NMC.
Desirable Knowledge & Qualifications
* Evidence of post-registration study & training (e.g., POVA).
* Child protection.
* Intermediate life support.
Experience (Essential)
* Pre-registration evidence in direct nursing care.
* Experience of providing holistic nursing care.
* Interest in developing nursing skills.
Desirable Experience
* Clear understanding of the clinical governance framework.
* Implementation of evidence-based practice.
Skills & Competencies (Essential)
* Clear understanding of contract of care.
* Ability to document details clearly and accurately.
