An exciting opportunity has arisen to undertake a lead role of Diabetes Specialist Nurse in a secondary care inpatient/outpatient service. The post requires a motivated individual to work 18.75 hours a week within the diabetes team employed by NNUH as part of the T1DE (Type 1 Disordered Eating) Project.

Please note that this advertised position, which is part of occupation code 2233, does not meet the UKVI eligibility requirements for a Skilled Worker Visa (this includes if you are already in a sponsored post and looking to change employer). This is due to the role being part-time and not meeting the minimum salary requirements. Therefore, the Trust would not be able to issue a Certificate of Sponsorship for this role.

* To provide clinical leadership and expertise in the diabetes speciality by providing information, support, advice and specialist knowledge to patients, their families and carers, and to act as a resource to the multi-professional team. To participate in the coordination of patient care to ensure continuity throughout the patient’s care pathway. To be pivotal in review and development of the T1DE service, and to reflect the needs of the patient group.
* To liaise closely with the health care teams of inpatient wards, outpatient department and primary care, including Consultant Physicians and GP’s. To demonstrate wide and in-depth experience of all aspects of the speciality, exercising clinical leadership and effective education within their sphere of practice. To monitor and improve standards of care through clinical audit, leading the development of practice through research.
* To lead on the delivery of Diabetes Specialist Nursing care within the hospital, predominantly, as part of the T1DE project MDT, providing clinical leadership and support, development of nurse-led protocols/guidelines and nurse-led initiatives.
* To develop and lead in the collection of relevant audit data to facilitate quality improvement, service development and national requirements.
* Provide evidence-based, expert care and education for patients with Diabetes with a particular focus on the patients accessing the T1DE service.
