Job Overview
The purpose of this role will be imperative in improving integrated pathways to prevent fragmented services that are efficient and effective. The role is specific to support those to improve health outcomes especially for those who continually consume alcohol where traditional structured treatment will not be appropriate. The role will require a peripatetic nurse led general health clinics within hostels, supported through general practice. The collective aim of the service is to help those with a range of health needs including drug and alcohol to access the appropriate treatment, and to address the physical and mental health of hostel residents. The service will provide essential health promotion to increase the number of residents engaging with services to address their health and wellbeing. Furthermore the aim of the service will be to reduce alcohol related hospital admissions, health inequalities and improve pathways with community and hospital services. The service should increase access to preventative and non-emergency health care; this will include health promotion, health checks and accessing mainstream services.

Main duties of the job
The team provide health and wellbeing assessments (H&WB) including a range of interventions to address health and well-being needs plus referral and facilitation into primary care and specialist services. The service also aims to provide training to hostel staff as well as offering client’s health promotion and general health screening.
The Peripatetic Nurses target drop-in/day centres/churches within the borough, providing a much-needed community and health screening service to the homeless, deprived and vulnerable people.
A large part of the health assessment delivered by the Peripatetic Nurse focusses on understanding the clients’ lifestyle and practices. Therefore, providing health promotion information and advice represents a considerable part of the work undertaken. The Peripatetic Nurse works within a motivational framework to assist clients in thinking about adopting healthier behaviours.
The nurse visits local hostels and does health and wellbeing nursing assessments with informed written consent from the client. The health and wellbeing assessments are given to the client/GP etc. highlighting concerns of health and appropriate referrals/ appointments made on the client’s behalf.

Detailed Job Description And Main Responsibilities
The aim is to provide a service specific to the homeless, rough sleepers and hostel residents that reside in the Milton Keynes. The role will include a range of activities addressing the resident’s health and well-being, the development of pathways into specialist services, training of hostel staff to managed residents out of hours, and improve the overall health inequalities via;
* Health promotion
* General Health screening
* Engagement in Primary Care services and facilitate access to organised health care

Person specification
Education
Essential criteria
* RMN or RGN
* Evidence of continuing personal development and post-registration training in the nursing care, assessment and treatment substance misuse clients.
Desirable criteria
* Diploma in Drugs and Alcohol Studies or equivalent qualification
* Training in Cognitive Behavioural Therapy
* Completed the Motivational Interviewing Training Module
* Completed the ‘Non Medical Prescriber’ course
Experience
Essential criteria
* Experience in Mental or general Health
* Experience in supervising junior staff
* Experienced in the supply and administration of Hepatitis vaccines
* Experienced and competent in the supply and administration of CNWL Trust Hepatitis Patient Group Directions
* Extensive clinical experience
* Case management experience
* Experience in undertaking Risk Assessments and Risk Management
* Teaching and Assessing qualification ENB 998 / SLICE or equivalent eg: mentorship
Desirable criteria
* Community Nursing Experience
* Experience of working in and delivering services to injecting drug users.
* Experience of teaching others and willingness to undertake Teaching and Assessing qualification within six months of commencing post.
Skills and knowledge
Essential criteria
* Sound clinical knowledge and understanding of current NHS policies and national drug issues/policies.
* Good verbal, written and presentation skills.
* Good interpersonal skills in order to communicate and negotiate effectively, both inside and outside the Trust
* Ability to work independently and to prioritise effectively
* Ability to assess and treat people with dual diagnosis
* Evidence of effective leadership and organisational skills.
* Evidence of the ability to work in a changing and stressful situation.
* Evidence of ability to teach/disseminate knowledge and skills
* Ability to treat service users with respect and dignity at all times, adopting a culturally sensitive approach, which considers the needs of the whole person
* Able to build constructive relationships with warmth and empathy, using good communication skills
* IT literate: competent using Microsoft Word and able to report clinical activity data on patient information systems.

* Experience of working with stimulant users and people with alcohol problems.
