The Medicine for Older People Department at East Lancashire Hospitals NHS Trust is looking to recruit an Advanced Clinical Practitioner in Frailty. This role will join an expanding team and contribute to the development and delivery of frailty services across the trust.
Responsibilities
- Undertake clinical and holistic assessment of frail elderly patients in the Older Persons Rapid Assessment Unit and across other clinical areas as needed.
- Provide guidance and expert advice to staff to ensure safe and effective care for frail elderly patients.
- Act as a clinical leader within the trust.
- Support and guide the ongoing development of frailty services across ELHT.
- Work closely with the wider multi‑disciplinary team to ensure a high standard of care.
Qualifications
- Registered healthcare professional with current registration (NMC or HCPC).
- Completed a Master's level ACP programme.
- Minimum of five years post‑registration experience in a relevant setting.
- Evidence of continuing professional development.
- Independent and Supplementary Prescribing.
- Advanced history taking and patient assessment or equivalent.
- Experience in leading a quality improvement project.
- Understanding of elderly medicine/frailty principles and the ACP role.
- Strong communication and interpersonal skills.
- Ability to manage a caseload independently.
- Proficiency in using electronic health records.
- Critical thinking and problem‑solving skills.
- Awareness of current healthcare issues and developments.
- Compassionate and patient‑centred approach.
- High level of professionalism and integrity.
- Commitment to personal and professional development.
- Willingness to adapt to changing needs of the service.
- Ability to manage priorities under pressure.
- Willingness to work extra hours on occasions and provide reasonable flexibility in terms of hours worked.
- Flexibility to move between work environments and rotate around clinical areas, and UK professional registration.
